| REFERRAL
AND ACCESS: |
Specialist Referral
through project Access (Medical Society of Sedgwick County)
and volunteer specialists (urology, orthopedics, cardiology, oncology, surgery,
etc.)

• Prenatal care
and delivery arrangements can be made for those who do
not qualify for other programs• Diagnostic tests (lab, x-ray, EKG’s etc.) at no charge to patient
• Hospitalization
• Vision and Hearing
• Dental Care Referrals
• SRS referrals on site |

PATIENT
QUALIFICATIONS:
To qualify for medical care at Guadalupe Clinic, you
must meet all of the following criteria.

• Be without health insurance of any kind (including
Medicaid or other medical assistance through SRS)
• Have an income within 200% Federal Poverty Level (will be verified with
proof of income)
• Must not be under the care of any other physician
• No Workman’s Comp or other insurance claims |
If you meet the above
criteria:
1. Call Guadalupe Clinic (316-264-8974)
for further information. Medical staff will then review
patient’s eligibility and schedule appointment.
Once patient is accepted:
1. Patient’s first visit at Guadalupe
Clinic: Patient MUST bring proof of income (all household
income) and will be asked for a $5.00 donation to see
a doctor at time of appointment.
POSSIBLE TYPES OF PROOF OF INCOME:
• 30 Days of paycheck stubs
• Letter from Unemployment office verifying amount and duration
• Letter from Social Security verifying you receive monthly disability
income
• Letter from whomever is supporting you stating they are currently your
sole support
